注意事項：

1、承租人遺失租賃契約書或租賃契約書滅失者，得申請補發。
2、線上申請後應補附租賃契約書遺失切結書，寄送所屬地區分署或辦事處辦理補發手續。

1. The Lessees, due to the loss or destruction of the lease agreement, may apply for reissuance.

2. After on-line application, an Affidavit of the loss of the lease agreement shall be replenished to the Region Branch or Office for the reissuance procedure.

一、依電子簽章法第4條第1項及第9條第1項規定，承租人同意以使用本系統產製具有電子簽章之電子文件，取代書面文件及簽名、蓋章確定之相關法律責任。

二、承租人遺失租賃契約書或租賃契約書滅失者，得申請補發。
1. In accordance with the provisions of Paragraph 1 of Article 4 and Paragraph 1 of Article 9 of the Electronic Signatures Act, the lessee agrees to use the electronic files with the electronic signature produced by this system to replace the related legal liability such as paper documents, signatures, and seals.

2. The Lessee, due to the loss or destruction of the lease agreement, may apply for reissuance.

一、本人同意貴署（含所屬分署、辦事處）依個人資料保護法第15條規定，基於國有財產管理之特定目的，於必要範圍內蒐集或處理本人之個人資料，並依同法第16條規定，依法定職務為必要之利用。

二、本人因租約書不慎遺（滅）失，特此切結並聲明原租約書作廢，切結事項如有虛偽不實者，願負法律責任，並無條件同意出租機關撤銷或終止租約，交還土地，所繳租金及歷年使用補償金等費用不要求退還。
1. The undersigned hereby agree that the NPA (including its subordinate Region Branches and Offices) may collect or process the undersigned’s personal information within the necessary scope for the specific purpose of management of the national properties in accordance with the provisions of Article 15 of the Personal Data Protection Act, and may make necessary use of the undersigned’s personal information according to the NPA’s statutory duties in accordance with the provisions of Article 16 of the Personal Data Protection Act.
2. I, due to an accidental loss (destruction) of the lease agreement, hereby waive and declare that the original lease agreement is voided. In the condition that there is any fraud or falsehood, I am willing to take legal responsibility, unconditionally agree to the leasing authority revoking or terminating the lease agreement and return the land(s), without requesting refund of the rent paid and the compensation for the use over the past years, or other expenses incurred.
